Consignee Letter of Assurance (End-Use Statement)

Dwyer Instruments, Inc. (Dwyer), including all its divisions, subsidiaries and affiliated companies, has a continuing commitment and obligation to comply with U.S. export laws and regulations. In the event that your order has an ultimate destination outside the United States, Dwyer requires that a "Consignee Letter of Assurance" form be completed in order to fulfill our compliance obligations. The completed and signed form can be returned by email orders@dwyer-inst.com, or by fax 219-872-9057. Early attention to this administrative matter will ensure that there are no shipment delays resulting from unavailable export documentation.
